Am I Really Stressed?

For each of the items below, write:  (1) strongly disagree; 
(2) mildly disagree; (3) neutral; (4) mildly agree; (5) strongly agree 

____1.  I   have recently experienced difficulty falling asleep at night.
____2.  I am more easily irritated now than a year ago.
____3.  I have less recreation in my life now than a year ago.
____4.  Over the past two years, I have had an increase in stress related 
             disorders such as headaches, high blood pressure, and gastric disorders.
____5.  I consume more alcohol now than a year ago.
____6.  I consume more stress related medications such as pain killers and 
             tranquilizers now than a year ago.
____7.  Often I feel that there is more and more to do in less and less time.
____8.  Within the past year, I have considered seeking some type of help for 
              feelings of stress.
____9.   I feel stressed much of the time.
____10. All things considered, my life is worse now than a year ago.

Scoring:
15 points and below - No Indication for treatment at this time.
16 - 20 points - Neutral 
21 - 25 points - Mildly stressed
26 - 30 points - Moderately stressed
31 - 35 points - Stressed
36 points or more - Very stressed
